Please fill out the following Volunteer Clearance Disclosure form so that we may perform a Volunteer Clearance Background Check. Background Checks are run by the District Office and are good for two years from the initial date of your clearance. Volunteer Clearance will be passed along to your student's school.

If you are a returning volunteer, feel free to confirm your Volunteer Clearance with your student's school or classroom teacher before submitting a new form.
